I here by join Ohio AFSCME Retiree Chapter 1184 and designate said chapter as my duly chosen and authorized representative to promote and protect my economic welfare to the extent authorized by law. I hereby request and authorize the deduction from my monthly retirement allowance the amount certified by the Retiree Chapter as the current rate of dues, including any increases or reductions in that amount. You may cancel this deduction at any time by sending written notice of cancellation to AFSCME or your retirement system. Cancellation will be effective no later than 60 days after receipt.
